Position description:
Provide skilled nursing care and psychological support to patient/family/caregiver as directed by the supervising RN Case Manager or On-Call Registered Nurse. Provide education to patient and/or caregivers regarding needed aspects of care to sustain the patient in his preferred setting of care. Provide timely documentation of patient services and care provided based upon plan of care. Identify other physical, psychological, social and spiritual needs for assigned patient. Work closely with all members of the Interdisciplinary Group to ensure all patient and family care needs are being met. Adhere to company policies and procedures and regulatory compliance requirements and directives.
Examples of duties include but are not limited to:
Conduct nursing assessment, including physical and psychosocial aspects of patient, family, and caregiver in a thorough, accurate, and timely fashion. Assessment to be completed and documented before end of 3rd hour of each shift.
Consistently collaborate with the Case Manager/On-Call RN to plan and implement the medical and nursing plan of care. Adheres to the plan of care and collaborates with the CM/OC RN should the need for change to plan of care arise.
Communicate, in writing, in patient's chart at a minimum of once an hour. Documentation will include interventions and patient/caregiver response to interventions and/or education. Will give thorough verbal report to the oncoming CC nurse which includes the previous shift's care needs, interventions and problems in order to ensure continuity of care. Shift notes to be completed prior to end of shift.
All documentation will be legible, complete and accurate to reflect the care given to the patient.
Will notify CC Coordinator and CC Manager the time when you start a case and the time when you leave a case that has been discontinued. Notification either by phone or text message (after hours).
Facilitate the implementation of nursing interventions and appropriate preventative and therapeutic nursing procedures for problems that occur during the shift.
Provide appropriate information and explanation to the patient's families. Facilitate patient and family education to promote continuity of care and optimal patient/family outcome.
Participate in the orientation of new personnel and assist with field training as needed.
Teach patient, family, and caregiver disease processes and self-care techniques. Provide medication, diet and other instructions as ordered by physician and recognize and utilize opportunities for health counseling with patients, families, and caregivers.
Offer constructive suggestions and be supportive of care and efforts of other CC team members.
Maintaining confidentiality in all areas.
Obtain report from CC nurse and/or CM or on Call RN staff prior to start of shift.
Notify CM or OC RN of patient death in a timely manner.
Will submit weekly Time Sheets to the CC Team Manager by 10 am every Monday unless otherwise directed by the Manager. Time sheets will be complete, legible and accurate at to drive times, time on a case and mileage to and from a case.
Relates complete and pertinent information in verbal and written communication.
Position
Qualifications:
Graduate of accredited practical or vocational nursing program.
Minimum of one year of general nursing in community heath, long-term care or medical/surgical experience required. Minimum of 6 months hospice experience preferred.
Ability to work within an interdisciplinary setting.
Understanding of hospice philosophy, principles of death/dying, and the ability to advise the caregiver in proper management of symptoms. Understanding of the needs of the terminally ill.
Good physical stamina and mental health. Comply with company policy regarding annual TB testing or chestxay/TB Questionnaire and Health Questionnaire.
Compliance with accepted professional standards and practices.
Valid driver's license and an automobile that is insured in the current state and in good working order.
Demonstrate excellent observation, assessment, nursing judgment and communication skills.
Ability to provide accurate written documentation in a timely manner.
Self-directed with the ability to work with minimal supervision from the on call Registered Nurse, RN Case Manager or CC Manager.
Flexible and cooperative in fulfilling all obligations. Dependable in meeting professional obligations of attendance and timeliness in reporting for duty as scheduled.
Satisfactory criminal background check and drug-screen.
Maintains current CPR certification.
Completes initial and annual mandatory education and training as defined by the company.
Complies with all aspects of HIPAA requirements and regulations.
